Requesting a copy of your report
If you believe RAI has performed an employment background investigation on you and you would like to receive a copy, please click on the link below to complete this form, indicating that you wish to receive a copy. The report will be mailed to you at the address you provide. We prefer not to email reports due to privacy concerns. So please ensure the mailing address you provide is current and accurate.
Disputing your report information
If you have received a copy of an employment background investigative report and wish to dispute the accuracy or completeness of any information in the report, please click the link below to complete the form, indicating which part(s) of the report you are contesting, the reasons you disagree with that part of the report, and any other information you deem relevant to your dispute.
You may submit documents in support of your dispute by faxing them to 440-892-9439, attention General Counsel Office or sending via regular mail to RAI General Counsel Office, 27999 Clemens Road, Westlake, OH 44145.
Please also include any documents you believe support your dispute.
We will promptly investigate your dispute and in most cases, advise you of the results of our investigation within 30 days of receipt. It is important that you include as much detail as possible in order for us to promptly investigate your dispute. All responses from RAI will be through U.S. Mail so please ensure you provide an accurate and current address.
